prevent impending disaster.  But the combat Navy Commander
has as his “eyes and ears” only the information sent to him
through friendly communication channels and those meager
facilities within his ship.  Every second, every minute, in the life
and actions of a ship are critical.  Four historical incidents, which
led to a Navy Court of Inquiry, are examined – the stories of four
